New resource on CT health equity

Making data actionable, the Connecticut Health Equity Dashboards offer a single source of detailed information on important health and social determinants at the town and neighborhood levels. Created by the state Office of Health Strategy, the site compares dozens of metrics across chronic care management, burden of disease, complications and poor outcomes, access to prevention, and the rising cost of care.
For example:
- Avoidable ED rates by town average 149.6 per 1,000 state residents but varies from a high of 240 in Windham to a low of 56 in Morris.
- Residents with three or more chronic conditions, such as asthma, chronic kidney disease, and diabetes, averages 26% for the state but varies from 42% in Lyme to 15% in Darien.
- Preventable hospitalizations average 8.6 per 100 discharges across the state but varies from 12.1 in Putnam to just 3.0 in Stonington.
- Interestingly, while Putnam is highest in preventable hospitalizations, they have the lowest rates of all-cause readmissions. (Feels like there is a story there.)
You can find data by rankings or mapped out. You can find Area Deprivation Indexes, a measure of social needs, by neighborhoods within towns. You can find uninsured and disability rates by town. There is also a Resources section for information and services. An excellent tool to identify local needs and target solutions.
